Here you provide the information that is essential for the medical professional who must decide what the priorities are, in case you are hospitalised unconscious as an emergency. (E.g. pacemaker or similar).
Is written on the front of your member card along with all the necessary medical information.

Can also be found on the blood type card.

If you have a blood type card you will find the information there. If not leave blank.

Is not an input field, but automatically indicates which donors you can receive blood from.
